Our main website address is https://www.divinepoint.com, and we are located at 1840 Post Road, Suite 2, Plover, Wisconsin 54467.  Our incorporated name in the State of Wisconsin is Divine Point Acupuncture, LLC., and we are Doing Business As Divine Point Acupuncture.

The Owner and Principal Provider of services is Jessica Lobner, licensed as an acupuncturist in the State of Wisconsin.

What Personal Data We Collect

We collect two types of Patient Data about you, as follows:

1. Your Contact Information.  We collect data that we need to communicate with you and to identify and associate treatment data that we acquire while treating you or consulting with you.
2. Specific Healthcare Information.  This is the data that we collect as part of our ongoing treatment plan with you.

We may also collect any information that you provide us when sending us a message through our contact form or by direct email.

Where We Store Your Information

If you send us a message through our contact form or if you email us directly, that information is stored on our mail system servers provided by Google.  Please refer to privacy policies that are published on the Google websites.

Once you enter our Booking and Appointments system, your information is stored on servers provided by our healthcare information management and booking provider, Jane.  The information collected by Jane includes, but is not limited to, the following:

• Your contact information
• Your health information necessary to provide you services.

HIPAA Compliance

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal law that required the creation of national standards to protect sensitive patient health information from being disclosed without the patient’s consent or knowledge.  The US Department of Health and Human Services (HHS) issued the HIPAA Privacy Rule to implement the requirements of HIPAA.

Technology providers that use or store private patient information must protect the privacy of that information in order to be identified as HIPAA-Compliant.  Our healthcare information provider claims HIPAA Compliance and offers the text of how in This Document.

Cookies

When you use our website or any of our provider websites, cookies may be read from, and stored in your web browser.  No patient data is ever stored in cookies, and we use these cookies to provide services to you and to make your navigation of our web site easier.

Articles on this site may include embedded content (e.g. videos, images, articles, etc.).  Embedded content from other websites behaves in the exact same way as if you has visited the other website.

Analytics

We do not share your personally-identifiable data with anyone.  We may anonymously identify where you are and track that anonymous data to give us a sense of where patients are located in our treatment area.

What Rights You Have Over Your Data

At any time, you can contact us to request a copy of all the data that we currently have for you on file.  You can also request that we completely remove and delete your information from our systems.

Where We Send Your Data

We do not send your information to anyone without a properly-served court order instructing us to do so.
